Permissions & Emergency Care

Emergency Veterinary Authorization *If I cannot be reached in an emergency, I authorize Cujo & Co. LLC to seek veterinary care reasonably believed necessary for my pet. I understand I am responsible for veterinary expenses incurred for my pet.

Family Home Acknowledgment *I understand that Cujo & Co. operates from a family home and that children may be present on the premises during my pet's stay. I understand interactions are managed appropriately and separation can be provided when needed.

Photo & Social Media Permission *Yes, Cujo & Co. may photograph/video my pet and share the content on its website and social-media pages.No, please do not post photos or videos of my pet publicly.

Declining public posting does not prevent your pet from receiving services. Personal information will not be included publicly without separate permission.

Pet Service Agreement

By signing below, I acknowledge and agree that:

• I have provided accurate information about my pet's health, behavior, medications, allergies, and care needs, and I will disclose any known aggression, bite history, contagious illness, or other condition that could affect safe care.

• Cujo & Co. operates from a family home and children may be present on the premises. Interactions between children and guest dogs are managed appropriately, and separation may be used when needed for safety and comfort.

• Dogs may interact with other dogs during daycare or boarding. Even with reasonable supervision and care, normal pet activities can involve risks such as scratches, minor injuries, illness, escape attempts, or unexpected behavior.

• Cujo & Co. may separate dogs, modify activities, or contact me for early pickup when reasonably necessary for safety or my pet's well-being.

• Pickup and drop-off are by appointment between 7:00 AM and 7:00 PM. Daycare is $35 for up to 12 hours; stays beyond the daycare window may be charged the applicable overnight rate. Overnight boarding is $50/night and holiday boarding is $60/night.

• I am responsible for providing sufficient food, medication instructions, and accurate emergency/veterinary contact information. Medication will be administered according to the instructions I provide.

• I am responsible for charges for approved services and for veterinary expenses attributable to my pet. Payment instructions are provided after a reservation is approved.

• A submitted booking request or registration is not a confirmed reservation. Cujo & Co. confirms reservations separately.
